Below are the links to our forms. Instructions are provided respectively on each form’s page.

• Cremation Authorization – The cremation authorization is needed to complete the cremation process; we must have the legal next of kin sign this form and hand deliver or fax it to us.
• Death Certificate – The death certificate information is the form we use to submit the information to the State of Minnesota. Death certificates can be ordered from any county in Minnesota (we use Olmsted County, Rochester, MN). The charges for death certificates are currently $13.00 for the first copy and $6.00 for each additional copy ordered at the same time. When using our Simply Cremation Plan, Minnesota residents will be required to obtain certified copies of the death certificate on their own.
• Obituary Form - Used for submitting an obituary to the newspaper(s) of your choosing.
